See what’s working

Patients can choose to share their structured data before or during visits, so time together is more focused, more productive, and more personal.

Give patients more than advice. Give them tools.

🔍 Can I see my patients’ data?
This is a patient-centered tool, so individual data stays with the patient—unless they choose to share it with you. Some patients may bring in their trends or assessment scores to discuss during a visit, which can support more focused conversations.

We don’t send data to the EHR or add to your documentation load—no new inboxes, no extra dashboards to check.

If you’re using the app with 30 or more patients in a specific population (e.g., asthma, ADHD, depression), we can provide de-identified, aggregate insights to help you understand patterns and impact. Reach out to explore this option.
What conditions does the app support?
It’s designed to support a wide range of chronic conditions, including:
  • Asthma
  • Dry Eye
  • Depression
  • Anxiety
  • Chronic Pain
  • Migraines
  • And others
It’s also effective for wellness and habit-building.
Is the app HIPAA-compliant?
Yes. We use secure, encrypted data storage and follow best practices for handling protected health information. We never sell or share user data.
Can I customize the app for my practice or organization?
Yes. We offer two options:
  • Branded Access: Change the logo, name, and color scheme. Have your own app in the Apple App Store and Google Play
  • Custom White Labeling: Modify features, workflows, or content
Learn more about white-label options →
Can I use this app in a research or pilot program?
Absolutely. We’ve supported multiple pilots and real-world evidence programs.

Get in touch to discuss onboarding, outcome measures, and reporting capabilities.
What support do you offer?
We provide:
  • A provider toolkit for patient onboarding
  • Optional email support
  • For large-scale deployments: account management and deployment support
How long does it take for my licenses to become active?
We'll send an email when your licenses are validated and released. It can take 1-2 business days, usually much sooner.

➕ What if I want to add more licenses to an existing code?
You can upgrade your bundle at any time. When you move to a larger bundle for the same code, we’ll apply a prorated credit from your current plan toward the new one. Your code stays active, and patients already using it won’t be affected.

Need multiple codes?
Each access code is tied to its own license bundle. If you want to use multiple codes (for example, to track usage across different programs or providers), you’ll purchase a separate bundle for each one.
🔄 How are codes recycled?
Each code bundle gives you a set number of licenses. A license is only counted when a patient is actively using the app.

If a patient stops using the app for 30 days, that license is automatically released and becomes available again—so your bundle is always being used by active patients, not sitting idle.

This helps you get the most value out of your bundle, and you can always check your available licenses in the dashboard.
🤝 How does this impact my relationship with patients?
It makes chronic care management a team effort. By giving patients tools to reflect, track, and act on their own health between visits, the app supports more meaningful conversations when you meet.

Patients feel heard and empowered, and you get a clearer window into what’s happening day to day—without needing to chase down journals or rely on vague recall.
🧩 Will this fit into my workflow?
It’s hard to say—every provider is different. Some clinicians naturally weave tools like this into patient conversations; others find it harder to integrate.

We suggest trying the app yourself first. See if you can imagine describing it during a visit or including it in your follow-up routine. If it clicks, start with a small bundle of codes and see how your patients respond. There’s no long-term commitment, and simple, inexpensive experiments can expose practice enhancing workflows.
💡 What’s the benefit to me as a provider?
This app gives your patients a structured way to engage in their own care—especially between visits. You’ll know they’re supported with:
  • Daily check-ins and nudges that reinforce your advice
  • Biweekly outcomes data (via validated assessments) they can bring to their next visit
  • Trends that can help guide more focused, informed conversations
It’s a low-effort way to extend your impact between appointments—and help patients take ownership of their chronic condition management.
📊 Can I manage my codes and track usage?
Yes! You’ll have access to a code dashboard where you can:
  • See how many, and which, codes you’ve purchased and how many are in use
  • Add more codes as needed
  • Cancel or deactivate codes
  • Track activation trends over time
It’s a simple way to stay in control—no guesswork, no spreadsheets.
